Remanufactured Starter killed flywheel?

Hi everyone, I’m hoping to get some thoughts on this.

First off, my rig is a 2001 tacoma 3.4 manual.

Two years ago I put in a Ultima started. A remanufactured one from Oreilly. Not more than a year later my clutch/flywheel blew on me. I admit I was abusive to it. Last week my started started making a gnarly noise. I maybe started the car 10 time before I decided to pull the starter and inspect. Sure enough half the bolts on it had partially walked out.

Looking it over, my guess is that the gear was not fully engaging the flywheel. Now I am unsure of what I’m looking at. Is this new flywheel also shot? Image attached. Note the notching on each cog. Is this normal? Or is this caused by the starter gear not engaging?

Looks to me the starter was engaging the flywheel correctly. Note the shiny places on the flywheel. The noise looks like a starter bolt backed out and scrapped along the edge of the flywheel. Its been a month since your post. Was this the issue or something else ?
