Get two bowls and fill them with white vinegar. Close the truck up and leave it alone for two or three days. Did this to get the stench out of my house.

Depends on your skill level, time frame and budget. I bought a smokers car once and I replaced the carpet and headliner. Those two things really made a difference. you could also take the door panels off and really clean them good, inside and out.
