There is a Toyoyta dealer selling them on Ebay, I got mine for $201 shiped. It came with the mounting hardware.

I bought the factory one when I purchased my truck, look for a aftermarket one the new law made Toyota make it with 45 degree angle from the headlights and it is near useless I cant fit my full size dirt bike in and use the extender the exact reason I got the extender.
